We believe the best care starts with listening. Our person-centered planning process begins with your story, your culture, and your goals. Together, we build a plan that might include Homemaker Personal Care, respite, community integration, behavioral support, or vocational habilitation, always shaped around what matters most to you. We adjust supports as life changes, and we communicate closely with families and case managers so that everyone feels informed, respected, and part of the same team.

Our Care Leader
As our founder and care director, this experienced Certified Nursing Assistant has spent more than 20 years supporting individuals in their homes across central Ohio. Beginning as a Direct Support Professional, they learned that listening closely and honoring each person's routine makes all the difference. Today, they guide our licensed, Medicaid-certified team with that same hands-on wisdom. They work closely with families, case managers, and staff to create safe, realistic care plans that protect dignity and independence. Whether someone needs senior care, disability support, or help after a hospital stay, they make sure services stay person-centered, respectful, and reliable, day and night.
Community Support Coordinator
Our Community Support Coordinator helps bridge home care with meaningful life in the community. With years of experience in community integration and vocational habilitation, they plan safe outings, volunteer activities, and day programs that match each person's interests and comfort level. They also help organize non-medical transportation and coordinate with Unity We Stand Together so that individuals with limited income can access food, clothing, and other essentials. Families describe this role as a steady partner who checks in often, explains options in simple language, and treats every person like family while respecting cultural backgrounds and personal goals.
